- Permission for Care
- Individual Care Plan
- Medication Authorization
- Preschool – Permission to Leave Licensed Center
- Kindergarten – Gr. 5 – Permission to Leave Licensed Center
- CIS (Certification of Immunization Status)
- CIS (Certification of Immunization Status) Opt-Out

Instructions: If the form is a pdf, open each document, “save as” to your desktop (or click “open in desktop app) and title your student’s name/grade, complete the document, “save” and return to XDC as an attachment to an email to Mrs. Overby, roverby@st-johnschool.org.
